Active members of CASE are eligible to participate in a Group Life Insurance Plan and or Long Term Disability Insurance underwritten by The Standard. The summary of benefits and the enrollment forms are here for both. New Full CASE members have a short window I which to enroll and will have their own "Open Enrollment" period without needing to complete the Medical History Statement (they will receive Guarantee Issue).

Current members may also apply at any time; they will be considered "late enrollees" and will also need to submit a completed Medical History Statement form.

All forms will be faxed to the administrators, Nancy Bond Insurance Services, and the member will retain their originals. The fax number is 626/599-8579. Confirmation of receipt and acceptance will be given to the participant.
