SCIF Members
Employees who work for the State Compensation Insurance Fund are eligible to participate in their group Long Term Disability Insurance policy. To be eligible you must work at least 30 hours per week and be active at work when you apply. Please complete the enrollment form, the auto debit authorization form and the medical history statement and then fax these to us at 626/599-8579. Once you have been approved, your coverage will begin on the first of the following month. Payment is done via EFT from your bank account on the 5th of each month. Please see the SCIF LTD Summary for a description of the details on this plan. The Standard underwrites this excellent policy that has been in force since November 2001.
